[Cutaneous metastases of renal cell carcinoma]. / Metastasis cutánea de carcinoma de células renales.

Renal cell carcinoma has an unknown evolution. We report a case of a man with a skin metastases from renal cell carcinoma and an unfortunate result, five years after its radical surgical treatment. We review the literature and emphasize the need of a long and exhaustive surveillance in these patients.

[Acute renal infarction. Report of 2 cases]. / Infarto renal agudo. Presentación de dos casos.

Diagnosis of acute renal infarction is often delayed or missed due to both the rarity of the disease and its unespecific clinical presentation. Definitive radiologic diagnosis often requires invasive procedures such as angiography. Computed tomography (CT) may be useful in diagnosing acute renal infarction and is considered the standard of reference for diagnosis of acute renal infarction, with the advantage of being non-invasive and potentially available 24 hours a day. We report two cases, in the first case we show a segmentary renal infarction; at the second one we report a case of complete renal infarction. Presentation, diagnosis and therapeutic aspects are discussed.

El diagnóstico del infarto renal agudo es frecuentemente tardío o inadvertido, debido a lo poco común de la enfermedad y a la inespecificidad de su presentación clínica. El diagnóstico radiológico definitivo requiere en ocasiones procedimientos invasivos como la arteriografía. La tomografía axial computerizada (TAC) es útil en el diagnóstico del infarto renal agudo, y es considerada en la actualidad como la técnica de referencia, con la ventaja de ser una técnica no invasiva y estar disponible en cualquier momento. Hydatidosis in our country an important health problem because of its high prevalence. We present a case of renal Hydatidosis in which we point out the renal single location of the disease and emphasize that MRI helped us to find a correct preoperative diagnosis.

[Bilateral candida bezoar of the upper urinary tract]. / Bezoar candidiásico bilateral del tracto urinario superior.

Fungal urinary tract infections are due to candida albicans as first responsible germ. This sporulated oval fungus divides itself by gemmation and produces pseudohyphas when it is spread in culture mediums, organic tissues or their exudates. The most frequent predisposing factor is diabetes mellitus. Others predisposing factors are: long antibiotic administration, corticoids, immunosuppressors, neoplasias, neurogenic bladder, and catheter or foreign bodies into the urinary tract. Bezoar formation is a rare complication that sometimes produces obstructive uropathy. Although Candida albicans is the most frequent observed germ in bezoars, Candida tropicalis, Penicillium, Aspergillus, Mucor y Turolopsis have been found as well. We present the case of a diabetic patient with a bilateral bezoar formation into the upper urinary tract due to Candida albicans. Clinical features, diagnosis and treatment are described emphasizing in the are bilateral onset.

[Uro-andrologic alterations in Wolfram syndrome]. / Alteraciones uroandrológicas del síndrome de Wolfram.

The association of diabetes mellitus, diabetes insipidus, neurosensory changes and optic atrophy is known as Wolfram's syndrome. Herein we report on a patient with hypogonadism, pyelocaliceal ectasia and anejaculation. Although associated urological disorders have been reported before, this is the first case of anejaculation coexisting in the same patient reported in the literature. The possible etiopathogenesis is discussed.

[Intermittent aspiration in the treatment of urinary fistulas following cutaneous ureteroileostomy]. / Aspiración intermitente en el tratamiento de la fístula urinaria tras ureteroileostomía cutánea.

Two patients who had developed a urinary fistula following cutaneous ureteroileostomy are described herein. The foregoing condition was successfully treated conservatively with intermittent aspiration via the stoma. This treatment modality was generally performed using regular periods of application and aspiration pressure. The common complications of cutaneous ureteroileostomy are briefly described. Attention is focussed on urinary fistula, an uncommon complication, and the possibility of resolving the foregoing condition in a short period of time by aspiration via the cutaneous stoma before resorting to endourologic or surgical alternatives.

[Vesico-uterine fistula. Conservative treatment]. / Fístula vesicouterina. Tratamiento conservador.

We report on a case of vesicouterine fistula following cesarean section in a 26-year-old patient. This rare complication can be initially treated noninvasively with contraceptive steroids combined with other conservative therapeutic methods such as placement of a bladder catheter and electrocoagulation of the borders of the fistula.

[Treatment of inflammation processes of the lower urinary tract with dimethyl sulfoxide]. / Tratamiento de los procesos inflamatorios del tracto urinario inferior con dimetil sulfoxido.

We present 8 patients treated with Dimethyl-sulphoxide, suffering from inflammatory processes of the lower, urinary tract which include: interstitial cystitis, urethral syndrome, incrustation of the prostatic cell, actinic cystitis and chronic prostatitis. All the patients have shown an improvement in their symptoms which, along with the absence of side effects and its easy administration, means that the said drug constitutes a means of therapy to be taken into account in the treatment of these inflammatory processes, following the consideration of more aggressive therapies.

[Injuries of the ureter caused by gynecologic surgery. Experience in the treatment of 42 cases]. / Lesiones traumáticas del ureter condicionadas por la cirugía ginecológica. Nuestra experiencia en el tratamiento de 42 casos.

These operations, in which the risk is essentially ureteral, must be carried out in a urological rather than gynaecological way; the said ureter must be sought after a permanent, ureteral probe has been placed before the operation. We feel that a vaginography is more useful than a retrograde pyelography for diagnosing ureterovaginal fistulae. Another important indication is the need for an intravenous urography to be systematically carried out in the postoperative period of all pelvis operations. Only I.V.U. enables the discovery in time of ureteral lesions which would otherwise remain undiscovered too long. We feel that nephrostomy is an emergency therapy in cases of anuria and septic shock with a urinary focus. It is a safety measure for preventing kidney deterioration in ureter lesions which have remained undiscovered for a long time. In pelvic ureter lesions and providing that there is no vesical retraction, we perform a vesical psoization along with the ureterocystoneostomy (1). When the ureteral lesion is bilateral and the elasticity of the bladder enables us to do so, we perform a ureterocystoneostomy with vesical bipartition and psoization of both vesical wings. In cases of extensive, bilateral ureteral lesions associated with vesical damage such as: vesico-vaginal fistulae and retracted bladder, we perform a ureteroileocystoplasty and vesical stretching.

We present our statistics and results obtained in the conservative treatment of hydronephrosis in the inferior polar vessel with a kind of operation with certain modifications which we consider to be of practical interest and determined by the following premises:--Disappearance of pain after the operation. --Functional reestablishment and anatomical involution of the deformity. --Conservation of all the kidney tissue and its vascularization. --No possibility of infection due to permanent drainage and catheters.
